I read some comments on an Australian forum today that implied that, because of high rates of HIV infection, having sex with a Thai girl (with or without a condom) was like playing russian roulette with a loaded revolver. The author of those comments made it very clear that in his opinion, having sex with hookers in Sydney brothels was much safer due to mandated AIDS testing, etc.As I read the comment, I had many thoughts, but among them was, “Lots of people are making comments based on newspaper headlines that are probably ten years old. The fact is that Thailand has really done a commendable job combating AIDS in recent years, and its probably one of the less-risky places in the world to have sex. There’s a high level of awareness about HIV and AIDS prevention in this country.”I was hesitant to write a reply on the Australian forum, because my thoughts might only represent a personal opinion, not be supported by the facts. So, I went looking for some facts about HIV & AIDS in Thailand, and the following information is taken from the AVERT website. AVERT is “an international AIDS charity” devoted to “bringing you information on HIV and AIDS.”The website had this to say about Thailand: “There are very few developing countries in the world where public policy has been effective in preventing the spread of HIV/AIDS on a national scale, but Thailand is an exception. A massive programme to control HIV has reduced visits to commercial sex workers by half, raised condom usage, decreased the prevalence of STIs (Sexually Transmitted Infections) dramatically, and achieved substantial reductions in new HIV infections.Thailand, though, is also a reminder that success can be relative. Its well funded, politically supported and comprehensive prevention programmes have saved millions of lives, reducing the number of new HIV infections from 143,000 in 1991 to 19,000 in 2003….By the end of 2005, national HIV prevalence was 1.4%, down from 1.8% in 2003 and more than 2% a decade earlier.”

By contrast, Australia has about 900 new infections per year, with the incidence of AIDS in the general population being about 1/10th of Thailand [for 100,000 people, 140 Thais are HIV+, 14 Aussies are HIV+]

Also, the biggest cause of infection in Oz is male-to-male sex.

In Thailand the biggest cause of infection is heterosexual sex, however “HIV affects more men than women in Thailand; the male-female ratio is nearly 3:1″

A little quick calculation on my fingers & toes tells me that this makes the incidence among females in Thailand only about 70 per 100K, or about half the national average.
You may already feel completely buried in numbers, but let me get two more off my chest; here are the two statistics about new infection that really blew my mind:

* 50% of infections were the result of heterosexual transmission in cohabiting partnerships

* 15% of infections were associated with commercial sex

So in Thailand, half the people who contracted HIV in the relevant year caught the virus FROM THE PERSON THEY LIVE WITH.

My personal experience tells me that the level of condom use in Thailand is very high WITH BAR GIRLS AND THEIR WESTERN CUSTOMERS. They both are very conscious of the risks.

But some Thai sex workers are getting infected anyway (from a customer or IV drug use), then going home and passing it on to their boyfriend/husband who isn’t using a condom.

So what’s my message?

I guess its to say that statistically, Thailand bar girls are probably a higher risk proposition for HIV transmission than your average Sydney brothel. But that risk is probably much less dramatic than people think, if their impression of HIV & AIDS in Thailand is based on newspaper headlines from five or six years ago. So, a pinch of salt is in order when hearing people talk about Thailand as a place of disease-ridden whores.

But, all the progress that has been made in Thailand could slip away as an invisible cost of the political situation here.

Since September, we’ve had former and current Army generals running the country. They seem to have their hands full with politics-101 and some of the finer points of running a country, like safe-sex-education, have probably disappeared from the radar.

I suspect we won’t realize that cost for a few years, until todays children and teenagers become adults without the top-of-the-mind awareness of safe sex practices that we seem to have today.

We each need to take care of ourselves, and it would probably be good if we all did our best to reinforce the strong message about condom use to the bar girls we have sex with as well.

I don’t know your opinions or experiences with safe sex in Thailand. Every bloke I have ever talked to says he uses a condom every time, but almost all of them have ‘a friend’ who says its easy to get a Thai girl to ride bareback. I’m not sure how ‘safe’ everyone is playing it. I’ve been tested 3 times in the past 14 months, most recently in mid-February. So far I’m free of any STDs.

On those occasions when I go short-timing I use a condom, but at home with my live-in faen I don’t. Maybe I’ll end up in that 50% category ~ contracting HIV from my cohabitating partner (or perhaps she from me!). It wouldn’t be the first mistake I’d ever made, but in some ways, I guess you could say it would be my last.

Thailand seems to be a safer place to have sex than it was ten years ago. Let’s hope that trend continues to improve. Personally, I think Thailand will need to get back to some sort of ‘normal’ government soon or risk the loss of all the progress that has been made in this area in recent years.

    My experiences have been after the first time with a professional gal, they do not ask about condom use and in many cases will say it’s “ok” not to use one…of course they do that just for “me only”. I’ve always politely rejected those offers.

    I’ve found the non-professional gals will never ask for a condom and some will be very upset about me wanting to use one. Even if they aren’t afraid of diseases, I don’t understand why they are not concerned about pregnancy.

    The BBBJ establishments scare me. In theory, they seem like a haven for diseases. But those places seem popular with people on this site. Maybe someone can clue me in on why they aren’t scared?

    Interesting and thought provoking post. I am glad WW sent it in since most people or blogs tend to ignore these subjects.

    I have always felt that the sex industry tends to promote safer sex than the casual dating or casual sex. For instance the numerous times I have hooked up with a regular girl in Hong Kong and Japan they have begged for no condom sex. Seemed weird to me. My friend in Japan says that most of the time the Japanese girls he hooks up with almost beg for no condom sex. Where my experience with bargirls is they always use a condom.     Yes, Thailand’s improved a lot over the years. And yes, condom usage *should* ensure your safety.

    Condoms can tear, slip off or be broken down by the “wrong” kind of lubricant though, and if you’re here for a long time you’re pretty much guaranteed to have that happen at some point.

    Bearing that in mind has helped me to forgive myself for the idiotic occasions on which I haven’t bothered wearing one.

    Still, no harm done – touch wood. Ha ha.

    Good points raised,particularly on the efforts within Thailand to limit the spread of HIV.
    You could argue at the moment that education is better in Thailand than the US.I recently had the misfortune to watch the film Brokeback Mountain and still trying to get over it !! Seems the message was that unprotected sex between consenting cowboys and a flock of sheep on heat milling round the edge is the way forward.Amazingly it won an Oscar to ensure the message was sent worldwide.Maybe it’s me but seemed to be a film with no point other than to promote homosexuality to the masses.
    Anyway,I’m off to Cassanova Bar…yee haaa!! :-)

    In my experience many, many old hands go bareback with bargirls regularly. Some will wear a condom if the girl insists but try to get away without. There seems to be some sense of immunity among some of them (“I don’t know anyone who’s ever caught it”, “It’s really hard for a bloke to get it”). Perhaps it’s just another symptom of the self loathing, sex addict, alcohol and tobacco abusing hopeless Thailand case. Little respect for themselves or anyone else, and little true love of their life. Plus pretty tough to keep it up too with all that rubber on, given their general condition and increasing inability to be sated by anything but ever more perverse fantasies.

    I remember being told in a Sydney STD clinic a decade ago that the “odds” of a male with no other STDs contracting HIV from a single episode of unprotected vaginal sex had been estimated at 800-to-1, but that for obvious reasons this wasn’t a number the docs liked to quote to patients – might undermine the safe sex message completely.

    But even assuming that’s true and HIV is “hard to catch”, what about everything else? Inconveniences like syphillis, gonorrhea, cylamidia. Incurable life-long afflictions like genital herpes and human papilloma virus (genial warts), now recognised as the primary cause of cervical cancer. Don’t understand myself why anyone would want to have unprotected sex with strangers. Or even acquaintances.

    I work in the health care field, and here is how I feel about it (my personal opinion). This is not medical advice, it is just my way of looking at it.

    1) I examine patients with AIDS almost every day. To protect myself I wear a glove. I don’t see too much difference between a finger covered by a rubber glove, and a cock covered with a condom. I don’t think I will catch AIDS in my work, and if I use condoms, I don’t think that I will get it from sex with women.
    2) Rubbers can break. That does raise your risk. Still, it is not very easy for a guy to get HIV from a girl, especially if your cock is not covered with sores. Fortunately, rubbers don’t break too often. Don’t fuck any woman on her period.
    3) With a condom, it is also very unlikely to get gonorrhea or chlamydia. Just don’t “dip” it in before putting on the rubber.
    4) You can get warts and herpes despite condom use. That shit can get on your balls even if your shaft is covered. Too bad. These diseases won’t kill you. If you can’t stand the heat, stay out of the kitchen.
    5) The disease that is kind of scary is syphillis. You can get it from a sore on the girl’s pussy, or if she has the rash of secondary syphillis- from anywhere on her skin. This too can attack you balls even if your shaft is covered. Syphillis is really bad. My best advice, inspect the pussy for sores. Get tested for syphillis now and then. (it is not too bad if caught and cured in the early stage)

    Overall, fucking bar girls is more dangerous than fucking girls not in the entertainment industry (although no girl, except proven virgins, are completely safe). Stay away from druggies, get away from girls who want you to go bareback, and rubber up every time. For me, I am more afraid of getting killed by a crazy taxi driver on the drive in from the airport, than from anything I can pick up from one of the lovelies.

    Any insiders have estimates of the percent of girls (or customers) that have diseases? I know friends that have unfortunately received a few from workers the bar scene giving bareback bj’s. But overall I’d estimate it as a small percentage overall.

    Eventhough many bars/gogos give customers the impression they regularly test their girls for diseases I think few do. Some gogo’s even declare on their websites that regular testing is done. I’ve asked many girls I trust about this but none has ever said she’d been tested.

    It’s downright scary out there folks. I’m not a long term resident of BKK (yet) but I’ve been here long enough to hear plenty of stories and meet enough real life examples to know that there is a frightening amount of barebackin’ goin’ on out there.

    For one, I know someone who contracted HIV here in BKK; most likely because of innumerable unprotected encounters with street meat.     To think that there are people out there who regularly jump from girl to girl without a condom or a thought for HIV (not to mention the other nasties) is astonishing. As for the guy who got it and continued to merrily attempt to infect the girls… a bullet is the only treatment for that sort of scum.     Farangnut, I went through with the aids test in the UK, & it is worth doing just to give you peace of mind. It is just the length of time it takes for the results to come back. 3 bloody months, & it feels like forever waiting. You get two women feeling your balls for cancer, the old stick down the eye, & numerous blood samples. Does anyone know why it takes so long for the results?

    My last two tests (one in Bangkok and one in Sydney) each took about 5 days to process.

    For me, three months makes me think about the ‘window’ of time it takes for the infection to appear… in other words, from the time of exposure until detectable in your blood can take 90 days, so a ‘negative’ test doesn’t cover any exposure you may have had in that 90 day window.

    I know I would happily pay 500 baht more if I had a way of confirming a girl had been tested in the last three months that I trusted. I’m not particularly worried about HIV as chances are miniscule as a raincoat wearer. I would still rather avoid the curable and the annoying-and-permanent STDs.     I have heard of 2 guys (both now living in Pattaya), who have HIV. One (an English guy) is pretty sure he got it in Thailand, the other (American) thinks he got it in the US.

    They have met each other around 5 years ago, both having decided to move to Pattaya, and they both have a mutual interest in regularly having unprotected sex with girls they pick up, and when possible they ejaculate inside them. According to them, most girls (of the type they get anyhow) have no problem with no condom, although some ask the “not to come inside”. They would usually do this 3-4 times a week, generally with new girls, but also with regulars.

    The English guy is employed as a real estate agent, the American guy does not work. Both look pretty healthy, and the English guy even plays in a local rubgy team! His reasoning on this issue include:

    1 that if the girls are stupid enough to take these risks, they already either have it, or will get it anyhow.

    2 His wife (yes he has a thai wife – a very local farm girl and former bargirl) was recently tested and she does not yet have it, so it can be that hard to catch. “Maybe my strain is not that infectous). He has been married to her for more than 5 years and she does not know his condition.

    3 – Treatment these days, means HIV is not really a big deal.

    4 – I already have HIV, so the worst that can happen is that I can get an STD. (He gets an STD approx every 3 months, which he also probably spreads, but he takes antibiotics to cure this, and also asks his wife to take the medecine in case he has infected her).

    I don’t have sex with prostitutes in Pattaya, but if I ever was going to, knowing what I know about these guys (and there might well be more guys like this), I would always wear a condom. And I feel really sorry for the girls they infect (although, it is of course the girls fault by being so stupid as to allow them to do this to them).

  27. The comment on Brokeback Mountain I found kind of amusing. First I’ll say I live in America. In general Americans have a lot of retraint in sex. And if a man doesn’t want to wear a condom, then most women won’t even be with them (this of course only comes with maturity, which has little to do with age).

    There are very low corners where you can find women who’ll do anything for money, and there are those that’ll worry about eating right, but don’t think they need a condom.

    But, in America we are lucky enough to be highly educated in safe sex. In High School some schools will have a class on safe sex, or sexual abstinence at the least.

    But there are those that just do not listen to good sense and decide it’s cool to have sex without it, and it ‘ruins’ the mood of it. I feel sorry for those who feel that way.

    And yes it’s a myth. Scientifically proven. And I honestly believe that your chances of actually having sex with someone who has aids is slim. Most people that know it tend to be very careful. At least here in America.

    The Best way to avoid getting aids is to be as protected as possible, know who your partners are, and who they’ve been with and who they’re with now. If your having sex with more than one person or someone you’re having sex with is having sex with more than one person get yourself checked at least once a year.

    Unlike what some foreigners think not all Americans choose to be unintelligent.

    I have been to BKK even to Pataya….I have been asked by almost every bargirl, who was new in the area from a village, had two jobs to support her mom and a ‘young’ brother, to go into the back of the bar. I agree with a writer above, all of you who are thinking about having sex with someone you don’t know, you should speak with someone who has HIV, AIDS, Syphillis etc. Just imagine, stuff growing out of you, bugs growing in you, the smell, the stink…..My advice, not as a doctor, but as a fellow human being…protect yourself. You can later kick yourself in the balls, head, back, front, anywhere….it just won’t go away.

  29. @Gee: I agree entirely. Nobody should ever have sex with anybody. It’s far too dangerous. A 1/80000000 chance of contracting an STD far outweighs the fun.

    On a similar note, I have decided never again to leave my apartment (as there is a 1/80000000 chance that I will be hit by a bus), nor to eat any food or drink any water (1/80000000 chance that I’ll get fatal food poisoning), or even scratch my balls (1/80000000 chance that I’ll accidentally puncture my scrotum with an errant fingernail and hemorrhage to death).

    Seriously. Condoms are obviously a good idea, and I don’t think anyone can seriously argue against using them, especially with bargirls. But let’s leave the scaremongering out of it, eh?

    The guys with aids are putting all of us at risk, as well as others you know.     The name I am using with this post is in reference to what I have — herpes. I got it many years ago from the fourth girl I slept with.     Yes, all of us take risk, however, we can reduce the odds against us by being smart. Wrap every part of your body in plastic and you will be okay. (a little on the lighter side)

    Brummie: The only way someone would know about these guys is if they are in the medical field treating these sob’s. Screw your oath and turn them in. They aren’t going around bragging about having aids. The one guys wife probably has aids but is also lying about it.

    I think the Red Cross AIDS testing office has moved though from the Rama 4 location to Rajadamri Road opposite of Lumpini Park area, but I am not sure exactly where on Rajadamri it is.

    A couple of years ago I saw a sign advertising delivery of a home HIV test kit in Bangkok for 1,000 Baht and they had a web site as well, but can’t remember the site now.

    Worth checking whether your home test kit can reliably detect the Asian subtype of HIV (subtype C). A friend told me that many of the home kits apparently do not. Subtype B is the version most commonly found in Europe and North America, so that’s apparently all some kits will detect.

    Take that with a pinch of salt obviously, as you should all tales that begin with “a friend told me”…

    I have also seen a home test advertised online which uses saliva instead of blood, but seems to detect the HIV-1 strain. Again, I don’t know much about it:

    I wanted to comment that I believe that the amount of HIV positive incidence has gone down a fair amount in Thailand over the last 10 years as noted above, much in part because of a man named Mechai who is responsible for promoting condom usage in Thailand and raising awareness amongst the locals.     What was always a question though with the statistics coming out of Thailand from the government on the number of HIV positive infections was whether the Thai government was reporting accurately and/or if the numbers were deliberately reduced to dispel the negative press about Aids in Thailand as Thailand has been known to do in the past with other statistics which could negatively impact the international opinion of the country.

    I assume the overseas agencies reporting HIV statistics for Thailand are honest with their figures though, but it is hard to know if their source of the figures is always accurate either.

    I believe it is a hard statistic to accurately report for a developing country in general, and that goes for other developing countries in the region like the Philippines, Cambodia, and other areas where there is an active sex trade, but hopefully, with the infection rate having gone down in general, that the numbers coming out these days are more honest and accurate.

    Hard to know really, but I would think a figure from the Thai Red Cross HIV testing clinic should be a good one, if it is available, as they could at least give statistics on the trend line of HIV positive test results over a period of the last 10 years.

    Thai Red Cross – Anonymous HIV Test Clinic Update:

    Clinic has moved from its former location on Rama 4 and is now located in a nice spacious building on Rajadamri at the intersection where it meets with Sarasin Road. It could also be referenced as being opposite of the Northwest corner of Lumpini Park.

    The staff are friendly and efficient. The place is clean and organized. It could do with a few more signs in English inside which would help direct people to the right sections, but otherwise it is fine. It would also be nice if some of the leaflets telling about AIDS and Hepatitis were in English too, but I guess their target group is the local population.

    They give you a questionnaire to fill out when you come in and which they say is for statistical data purposes, but it asks for your birth date and the first letters of your first and last name. You could put any information there though that you feel comfortable with. They issue you a numbered card which you can use for future testing and they also digitally finger print your two index fingers. Again, not so anonymous, but maybe they use it for verification on future visits.

    The testing takes about 45-60 minutes in all and you can test for Heb-B and syphilis too if you like. Total cost is 460 Baht for all 3 tests with immediate results. Results are provided verbally only in a room with nothing written down it seems.

    On the way out you are also welcome to grab a handful of free condoms, Thai sized of course.

    The clinic is open daily from noon to 7PM.

    @Cheapolay – First and foremost in the hospital environment there are standards in reporting any disease and they are ICD 9 or ICD 10 – being the lastest….

    I work in hospitals everyday in BKK and there are some “added” ICD descriptions (not part of the international “so so” standard) which save “face” to the patient when they are diagnosed with HIV, syphillis, etc.

    The government does not get accurate statistics about diseases related to sex reported by the private hospital sector here! I truly doubt they get accurate results from the Public hospitals as well.

    Another “great” example is Malaysia and I did a bunch of software work for them there to at least improve there statistical reporting related to diseases, doctor certifications, etc.!

    My point being, you cannot belive international statistics associated with a disease – bunch of crap – put a filter of at least 30% either way!

    Digital Fingerprints – fucking “big brother” attacks again – how do they compare with anything – is Interpol now involved with healthcare? This is all a bunch of bullshit – I go to receive healthcare – and I am fingerprinted – FUCK OFF!

    You go to a hospital and you get blood taken (DNA) what fucking good is a fingerprint — what SHITE!
